The notice must explain the purpose of the survey, clarify whether it's part of the health or general education curriculum, and provide the exact content of the survey questions. Parents or legal guardians have the right to opt their child out of participating in the survey. The school must provide a mechanism for parents or legal guardians to opt out, which can be received by mail or in-person. The act takes effect immediately.
